Here's one we may have all seen, in SQL Server's Enterprise Manager:

This is a known bug, and likely won't be fixed in SQL Server 2000.
Thankfully, there is an easy workaround. Switch the database view to something else, like Large Icons, then switch the view back to Taskpad. This will force Enterprise Manager to regenerate tabs.html, and any dependant HTML files.
And if the workaround is a pain, don't fret; SQL Server 2005 has a totally revamped graphical user interface.
